Completing your application?
At the time of writing private doctors must complete a downloaded copy of the application form on paper but dentists are recommended to complete the application online. Medical practitoners who are working under the NHS will have to complete the online application between October 2011 and December 2011. You can print out a copy of the draft application forms from the links above and use it to plan your final application. This draft form could also be used as a paper record
with the details of your application.
Compliance In the application you must confirm that you are compliant in 16 of the 28 outcomes.If you are not compliant you will need to provide a SMART action plan for compliance.
Confusing questions
There are some sections about human 'respecting and involving people and' equality, diversity and human rights'. Many members have found the questions difficult to understand.
